Life Choice: N-Acetyl-L-Carnitine
N – Acetyl-L-Carnitine is a favourite product of the health-conscious. In addition to being good for mitochondrial health (and, by extension, overall health), studies on Acetyl-L-Carnitine have been particularly promising in the areas of depression, infertility, and HIV.
Depression: In a double-blind clinical study, 28 senior citizens with depression randomly received either 500 mg of Acetyl-L-Carnitine or a placebo 3x/day for 60 days. At the conclusion of the trial, the patients who received Acetyl-L-Carnitine showed dramatic improvement in their depression, and no serious side effects were noted.
Infertility: Seminal fluid contains carnitine, which, when converted to Acetyl-L-Carnitine, helps stimulate mobility in sperm. Supplementation with one or both of these carnitines has shown to improve sperm mobility, along with potential sperm count and overall fertility. In one study, 60 infertile men randomly received either a placebo or L-Carnitine plus Acetyl-L-Carnitine for 6 months. In the end, the active treatment increased the number of forward motile spermatozoa and overall motile spermatozoa. In another study, active treatment with these two supplements in infertile men resulted in a significantly increased pregnancy rate, total sperm motility, and forward sperm motility, while the number of atypical sperm cells decreased. On a related note, 1 g of Acetyl-L-Carnitine taken 2x/day for 3 months appeared to be beneficial for Peyronies Disease, though the specifics of how improvement happened are unknown. Peyronies Disease is an ailment in which a male?s penis is abnormally curved due to scar tissue, which causes curved and painful erections.
HIV: A clinical trial involved 29 patients between the ages of 27 and 41 who had a history of previous intravenous drug use. Of these 29, 72% had decreased serum total and free-carnitine concentrations. These levels are almost to the point of carnitine deficiency, which can influence cardiac symptoms and muscle weakness in some AIDS patients, and can therefore promote further immunodeficiency. With 2 weeks of supplementation, L-carnitine improved certain elements of immune function in the AIDS patients. Acetyl-L-Carnitine specifically can improve symptoms and promote peripheral nerve regeneration in HIV patients who experience a side effect of peripheral neuropathy from nucleoside analogue reverse transcriptase inhibitors. Acetyl-L-Carnitine improves mitochondrial function, which is disrupted with the medication. In a clinical study, 21 HIV patients had peripheral neuropathy from their medication. They took 1,500 mg of Acetyl-L-Carnitine twice a day for an average of 14 months. Six months into treatment, evidence of nerve regeneration was seen. Twenty four months into therapy, these results continued or became more pronounced. The severity of the peripheral neuropathy decreased in 76% of the patients. Approximately four years later, 16 of these patients were available for a follow up. Of those, 13 said that their symptoms had improved either very much or moderately and all were continuing to take Acetyl-L-Carnitine.
